Description

High density lipoprotein (HDL) is the smallest and densest of the five main lipoproteins which, in addition to HDL, include chylomicrons, very low density lipoprotein, intermediate density lipoprotein and low density lipoprotein. PMID: 20431113 HDL has long been hailed as the 'good' cholesterol because it is able to remove cholesterol from arterial walls and deposit it in the liver.[http://www.nature.com/nrd/journal/v4/n3/full/nrd1658.html] However, it has been found that measuring the level of HDL in the circulation does not provide an accurate risk assessment of premature cardiovascular disease (CVD). There are five subclasses of HDL including pre beta-1, small alpha-4, medium alpha-3, large alpha-2 and very large alpha-1. High concentrations of the largest alpha-1 HDL provides the highest negative predictor for CVD.Measuring these subclasses gives a more precise risk evaluation for CVD than just the level of HDL. The optimal combination of HDL subclasses for low CVD risk is a high alph-1 and low alpha-4 and pre-beta. Increased levels of alpha-3 are associated with an increased risk of CVD.
