In serologic testing, D positive blood is easily identified. Units which are D negative are often retested to rule out a weaker reaction. The weak D phenotype (D^u) is characterized by negative reaction with anti-D reagent at immediate spin (IS), negative reaction after 37C incubation, and positive reaction at anti-human globulin (AHG) phase. Weak D testing is done on all prenatal patients and candidates for Rh immune globulin. Testing may also be done on Rh negative donors to ensure they are truly D negative as well as Rh negative candidates for transfusion.
